Start from what happened, not from who did it. Ask why until the answer is something you can change in the process, then agree one countermeasure and one owner.

Good whys

Facts, not guesses. Go and look before you answer.
Process, not people. "Anna forgot" is not a root cause. Why was it possible to forget?
Stop at something you can change. Sometimes that is at why 3, sometimes at why 7.
Test it backwards. Read from the root cause up with "therefore". If the chain holds, you are close.
A reminder is not a fix. If the countermeasure is "tell people again", ask one more why.
