8D Report

For the problem a customer saw: the eight disciplines of the Global 8D, from the first emergency response to the closing of the team's work. Contain within a day, find why it happened and why nobody caught it, prove the fix with data, and change the systems so it does not come back. Every discipline has its status and its date, and the days count against your target times.

The defaults are common practice in automotive and logistics quality: containment within 24 hours (D3 by the next day), the root cause within 10 days, the corrective actions within 14, implemented within 30, the report closed within 60. Use your customer's rules where they differ.

Does this problem need an 8D, and what protects the customer right now, before anyone knows the cause?

How do you stop the next bad delivery while the cause is still unknown? Sort every place the suspect stock can be, and prove that the containment works.

D4 · Root cause and escape point

Why did it happen, and why did nobody catch it? Two root causes, each verified: you can turn the problem on and off with it.

Put the actions in place, show with data that the problem is gone, and only then remove the containment.

Which systems change so that this cannot happen again, here or anywhere similar?

    • Update the FMEA. Add the failure mode and its cause, then re-rate occurrence and detection.
    • Update the control plan. The new check: what, how often, by whom, and what to do when it fails.
    • Update standard work. Change the instruction at the workplace, so the new way is the only way.
    • Train the people. Everyone who does the work, on every shift, with a sign-off.
    • Read across to similar products and processes. Where else can the same cause sit: other sites, lines, zones, products?
