Risk Assessment

The documented risk assessment of each work area or activity, the way the German Gefährdungsbeurteilung does it: who is exposed, the hazards from the eleven hazard-factor groups, each rated likelihood × severity on a 5 × 5 matrix, the measures in the STOP order with an owner, a date and the check of whether they work, and the residual risk. See the matrix before and after, the open measures, the reviews that are due and what the Safety Cross says the assessment missed, and print it with signature lines.

Hazard factor catalogue

The eleven hazard-factor groups of the BAuA handbook on risk assessment, with typical examples for the kind of site chosen above. Pick one to fill the form, then write it for your own workplace.

3 · Measures, check and residual risk

For every hazard: measures in the STOP order (substitution, technical, organisational, personal), each with an owner, a due date and its status; once done, the check of whether it works; then the risk rated again as it is with the measures in place.

No hazards in this assessment yet. Add the first hazard above, or .

Rating scales and risk bands

Rate the likelihood of the harm you rated, not of any incident: a fall from the dock is unlikely, a bruise from a roll cage is not. Rate the most likely serious outcome, as the work is done today.

Likelihood

  1. 1 · Very unlikely Practically never: not expected in years of this work, here or elsewhere.
  2. 2 · Unlikely Could happen, but not known here; perhaps once in several years.
  3. 3 · Possible Has happened in similar work or here before; about once a year.
  4. 4 · Likely Happens here now and then; several times a year.
  5. 5 · Very likely Expected: happens often, monthly or more, if nothing changes.

Severity of harm

  1. 1 · Negligible No injury, or a minor one that needs no first aid.
  2. 2 · Minor First aid or a doctor's visit, back at work within three days.
  3. 3 · Moderate Injury or illness with more than three days off (in Germany a reportable accident), fully recovered.
  4. 4 · Severe Serious or lasting injury, occupational disease, long absence.
  5. 5 · Fatal Death, or several people seriously injured.
Risk = likelihood × severity (1–5 each), in four bands

Above the high limit the risk is very high. The standard is 4, 9 and 16.

  • Low Acceptable as it is. Keep the measures in place and look at it again at the next review.
  • Medium Measures are needed: plan them with an owner and a date, and bring the risk down as far as reasonably possible.
  • High Measures are needed soon, before the work goes on as usual: look first for substitution and technical measures.
  • Very high Do not carry on as it is: stop or restrict the work until measures have brought the risk down.

The STOP order

  1. S · Substitution Remove the hazard or replace it: another substance, process, machine or way of working that is less dangerous.
  2. T · Technical Deal with the hazard at its source and separate people from it: guards, barriers, interlocks, ventilation, lifting aids.
  3. O · Organisational Change how the work is organised: rules, routes, rotation, rotas, permits, training and instruction, distance in time or space.
  4. P · Personal Personal protective equipment and personal behaviour. It comes last, when S, T and O are not enough, and it needs instruction and checks.
